This document explains our 3 stage referral process.
Agency Referral Form
This form should be completed by the referring worker before being emailed to
dutyworker@scotscare.com. Please remember more information is better than less as it will avoid any delays in dealing with the request.
General Application Form (to be sent with a copy of the Agency Referral Form)
Please ensure that your client completes and signs this Application Form then send it with a copy of the Agency Referral Form that you emailed. It will save time if you enclose recent proof of your client’s benefits/income (no more than 2 months old) and their Scottish connection (birth certificate, passport or photo driving licence)
Please note the pack will need to be ‘unzipped’ you can obtain a trial version of WinZip from http://www.winzip.com/downwz.htm if you do not already have suitable software.
